Looking for a podcast story

A few nights ago I was listening to a podcast story and it was really interesting, but I fell asleep before it finished and now I can't remember which podcast it was and I can't find it anymore.
I think it was an episode with things that go wrong" theme, and the story I was listening too was about an air flight where the windshield blew off because the mechsnic used the wrong type of bolts. It was illustrating the Swiss Cheese Hypothesis. Anyone happen to know which podcast this was? I listen to This American Life, RadioLab, ReplyAll, Freakanomics, Criminal, Planet Money...doesn't seem to be any of these. Please help me find the podcast so I can find out what happpeed! |

| It was the new podcast about Reddit stories. The episode is called something like no No No No Yes. I think the podcast is called Endless Thread. |
