Passport application question -- city of birth (hospital or where you live)?

I am applying for my children's passports. Place of Birth question: Am I supposed to use the location of the hospital (i.e. Falls Church), or the address where we lived when the children were born (i.e. Alexandria)?

They want to know where your children were born, not lived. In my case it was super easy because I was born in Philadelphia ( Temple Hospital) and we also lived in Philly.

location of hospital if that is where they were born. use the the city of your home if it was a home birth.

OP here. Thanks for the quick replies. I looked on the birth certificate. One of them says "Fairfax County Virginia".

But the passport application asks for the "City and State if in the U.S."

So, do I use Fairfax County, Virginia?

OP, if they were born at INOVA Fairfax, put Falls Church, VA.

What city is the hospital address located? Use that.

That's what I was going to do -- but that's the one that says "Fairfax County Virginia" on the Birth Certificate.

For the umteenth time, use the address of the hospital. Look up the hospital's website. Find the address. Find the city. Use that.

Don't be rude. I can read the question. The problem is that the birth certificate will not match my answer. The Birth certificate does not say anything about Falls Church. It says "Fairfax County". I know how specific these types of forms can be and I don't want to fill it out in a rush when I show up and they tell me "it doesn't match."

OP, I was born at INOVA. And since we always called it "Fairfax Hospital" while I was growing up, I have always written down "Fairfax, VA" as my place of birth. I had never even considered that I was wrong until reading this thread and realizing the address of Fairfax Hospital is in Falls Church.

I have a passport and a security clearance so clearly it's not THAT big on an issue....

Thanks... I'm all done filling out the application. It's all good.

For one kid, it was easy! We live in Alexandria, he was born at INOVA Alexandria, and the birth certificate says "Alexandria, VA".

For the other kid, it is more muddled: She was born at INOVA Fairfax, which is in Falls Church, VA; we lived in Alexandria; and the Birth Certificate says "Fairfax County Virginia". But, I went with Falls Church. I'm sure it will be o.k.
