Fairfax County - VA health form

Does a new VA health form have to be completed for each year? My oldest is a rising second grader and has a well child checkup next week, but I am not sure if that paperwork has to be done annually. Does anyone know?

Do you use summer camps? Some camps require that form to be updated every year, so you might want to have it filled out and sent to you anyway. Then you can scan it in and save it, and you'll have it for anyone who needs it.
