Can you start a 504 process over the summer?

Or does it have to wait until school starts again. Now have a diagnosis but school is over, and we'd rather not wait until fall in we can avoid it, we'd rather have something in place at the beginning of the year.

You can start the process any time. However to qualify for a 504 plan, you need more than just a diagnosis - the team has to find that there is a substantial impact of the disability on your child's ability to perform whichever tasks are impacted by the disability. At a minimum, your child's teacher from last year would need to fill out the input form, and since teachers don't work over the summer they may ask you to wait until the fall.

You can request a 504 plan now, but the unless your child's teachers (last year's) are available nothing will happen.

They take the information you give them, and then have to do their own analysis as to whether the disability is or is not affecting the child in the classroom. You should gather work product, teacher notes and so forth from last you so you have evidence of the disability's impact on your child.

Send an email and the evaluation information to the principal and/or guidance counselor (depending on your school's model) now and ask for a meeting as soon as school starts.

Consider starting the IEP process instead. It has legal deadlines that don't stop over the summer, so assessments will begin quicker. You can always "settle" for a 504. Also, you can email her teacher from last year and see if she is willing to fill out evaluations over the summer. Some are willing to do it.

What is the input form? Can it be found online?

No. Your teacher would receive it from the 504 coordinator. Don't go directly to the teachers.
