IUI #1 last month - 50mg Clomid + ovidrel trigger - 2 follicles, 16mm and 32mm, BFN
This month my doctor wants me on 100mg clomid + ovidrel. I am a little nervous about this increased dosage, as my goal is a singleton pregnancy. However, we do have MFI so that probably does play into it? Last IUI we had 12.5M motile sperm after the wash, which is the best we've ever had. Advice? Anyone else have similar response under Clomid and tried 100mg? |
| Given that you got two follicles this time, why does the doctor want you to increase the dose? |
That's what I'm wondering about!! Maybe to try and get us to 3 follicles given our MFI? |
|
^ Maybe. Either way, I would ask the doctor and express your concerns about multiples. A lot of this is chance though.
Best wishes. |

I am no expert, but it seems like a 32mm follicle is a bit too large, and a 16mm is on the smaller side. Could the 32mm be a cyst? Is there a reason you didn't trigger sooner when the larger one was like 22mm?
In terms of higher dosage, maybe they are just hoping for a couple more follies around the same size. |
When i went in for monitoring on CD 12, the follicle was already that big. This time we moved up my monitoring appt to CD10 to try and avoid it being too big by time of trigger. |

You should always understand why they are changing your meds. If you aren't sure, ASK! There is no reason to be wondering.

+ to this. I don't understand his reasoning for increasing your dosage. It sounds like you had an excellent response from 50mg. |
| Thanks for the suggestions to call. I did and we have been back and forth. My clinic said the rationale for increasing it was to "have more follicles" and my response was to understand exactly how many follicles they want me to have?? This is making absolutely no sense to me as everything that I've read says the desired number of mature follicles is 2-3. I'm <35 and hormone levels are fine. Not seeing the medical indication for the change. |
So it sounds like you only had one really usable follicle when you triggered. I imagine this time they are aiming for two and a "stronger ovulation" this time around. Of course, they should spell that out for you more clearly. A fair number of clinics don't want to go through details with a Clomid + IUI cycle because they focus on the IVF cycles. (I've done a lot of both, so I speak from personal experience.) |
| NP with a similar question and wondering what questions I should be asking. I started a cycle with 50mg Clomid but had to cancel the cycle (not due to medical issues). I kept going back every 2-3 days and the follicles were slow to grow. Second cycle was 100mg and I got two follicles. It still seemed like they weren’t as big as the Dr wanted but we triggered anyway. Negative test. They want to up my clomid for this cycle. I’m guessing it will be to 150 mg (only talked to office staff on the phone). Is this the point where it’s advised to switch medications? We are doing IUI because of MFI and I’m 37. |
With MF, you are wasting time with IUI. Do you have to do it before getting insurance to pay for IVF? |
I have no insurance coverage for IVF, so IUI is our only option. |
Sorry, I just realized you are responding to OP who posted in 2017. I'm the NP with a similar issue. |
| What RE uses clomid anymore? My told me letrozole was the better choice, but if your dealing with MFI it doesn't matter how many follies you have??? |
