| It just seems so excessive. Some camps and aftercare require it as well. Some camps are different state forms so can't re-use. Doctors charge $25+ a pop. With 2-3 kids, you're talking about upward of $200/year just on forms. |

No they don't. I've had kids in schools in five different states. They did not all require yearly health forms from the doctor. They all did require proof of immunizations on their state forms when we registered as a new student. |

The Universal Health Certificate must document immunizations, tuberculosis assessment and physical exam completed within 365 days before the start of school. Every child less than six years of age must be tested twice for blood lead poisoning. Testing must be completed, regardless of exposure risk, and documented on Universal Health Certificate. |

This is not common knowledge, if it is true. Neither our MCPS school's health employee nor our doctor, who has been filling out forms for kids for years, are under the impression that the requirement is yearly. However it's common knowledge that DC does have this requirement. |
+1 This is my experience from living in different states as well. Our previous doctor's office didn't charge to complete the form either. |

Can you speak for all VA camps? I just had to use a separate Va form for camp. |
