Finished "referral" (kid in pool)

Though I didn't do much on it, I do feel good having it done.

Dumb question here. If you say your kid is already in the pool, what is the referral for? I thought a referral was if your kid did not get in pool and you wanted them to be considered anyway.

It is optional paperwork to fill out and provide more supplemental information. Not required but permitted.

So, what is the supplemental information you are providing? Something other than the parent questionnaire?

I applaud your efforts, but not your terminology. A "referral" is simply what you do if your child is not "in-pool". The optional paperwork has nothing to do with a referral. A parent can refer and still not provide any supplemental information. You're not submitting a "referral", you're submitting a "packet of information".

Since the paper is titled "referral" the terminology is correct.
