It means they will cover up to 3 attempts whether it be a fresh or frozen cycle. Some patients will pay out of pocket for FET to save their 3 chances for fresh cycle. So if you don't get pregnant and have a live birth after 3 attempts, you would have to pay out of pocket for any attempt beyond 3rd attempt covered by insurance. However, let's say it takes 3 attempts to have successful pregnancy and live birth, and then in the future, you want a second child. Then the insurance would pay for up to 3 attempts for second live birth. If you hit lifetime max, then they would stop paying once you hit that threshold.
