Where to get custom slip covers for old Crate and Barrel couch?

I have a couch that is at least ten years old from Crate and Barrel. It's a slip cover sofa and while the couch and cushions are in great shape, the slipcover is not- faded and worn. Crate and Barrel does not sell replacement slip covers for this model anymore. I don't want to get rid of the couch, so what are my options? Are there reputable places that anyone would recommend that will make replacement covers that look nice?
