Clueless K parent: I just got the Student Information Sheet, what do I put on it???

I know I'll probably get totally flamed for this, but I have no clue what are my child's Social/Emptional/Behavioral needs or Work Study/Organizational Skills are??? He is good kid (no behavioral problems), beginning reader and good "out of the box" thinker... However, he is completely different kid in school vs home (better behaved, etc.)
Shouldn't his teacher be completing this profile and not me? I am not with him in school 5 days out of the week!
What do you guys put on the profiles?

If I were you, I would put "He is a good kid (no behavioral problems), beginning reader and good "out of the box" thinker" under Social/Emotional/Behavioral needs and "He is a completely different kid in school vs home (better behaved, etc.)" under Work Study/Organizational Skills.

They supposedly use these forms for planning classes for the next school year. They will already get your teacher's input so they are really looking for anything you'd like to add. I have a 1st and 3rd grader and I honestly don't think they give the form much consideration. But it can't hurt to put down if you think your kid thrives better under a certain teaching style or needs help with organization, for example. I wouldn't sweat it too much.

I think it is mostly an attempt to give you input into a process that you really hve very little input in. If there is a teacher for next year that you would prefer you can kind of mold your answers.. "Johnny does best with very consistent rules adn guidance" or Johnny is very creative and needs freedom to explore during the school day"

I'm a K teacher and we do not use these forms to make classes. I can't imagine a school that does, although they might. That would be a lot if work to sift through comments for 100 kids and make classes based on that. Once my class is made, THEN I look at the info sheets to get a better feel for my class and the individual students.

What you've written is exactly what you should write. It will be very helpful for his teacher.
