health forms for returnign PG public school students?

I know this sounds a stupid question but...
I'm used to completing health forms and immunization forms for my kids' private elementary school each year. What about public middle school students? I have a child who started PG public school last year. We completed all the health forms to get him into the school system. Now that he is returning to the same school, is there a health form to complete? Could someone send me the link? I googled "pg county public school health forms" but only saw what new students should complete. |
