I just had a DE miscarriage at 8 1/2 weeks after a strong heartbeat. I am strongly suspecting clotting or other immune problems, even though two pretty thorough panels in 2012 revealed NOTHING.
I will use Lovenox on my next transfer, but want to explore if there is a reason to try intralipids. IVIG is out of the question due to cost. Are intralipids for maintaining pregnancy, or implantation? Because I don't have a problem with implantation, I've been pregnant several times (OE before DE). I don't want to move forward without covering all my bases. Who in this area will do immune testing, and who may administer intralipids? |
| Dr Abbasi is the only one in the area that would do immune testing. I used Walgreens pharmacy for the intralipids. Transfusion. It was 450 dollars per visit, last did it in May. |

10:40, how did you arrange for the treatment? Who prescribed it? Did Walgreens give you the equipment to do it at home?
For which immune disorders are intralipids prescribed? I know I don't have an implantation issue, I get pregnant but it dies after the first heartbeat. If intralipids will help, I am more than happy to do them. As I understand it (research so far), you only have to do a couple of intralipid infusions. |
10: 40 poster - In my case, it did not help as i am still not pregnant. But I am a nut case and have a track record of 8 failed fresh IVF cycles. The last two were with intrallipids. I know others who have been successfull. It does not hurt to do it, at a minimum for a piece of mind, if your RE susspects immune issues. |
10:40 poster here again. Dr Abbasi prescribed the treatment; I called Walgreens to schedule an in-home visit. They provided the equipment. My immune issues were diagnosed by Dr Braverman. I did 2 Neupogen cycles with him, he also put me on steroids and lovenox. I could not afford to cycle with him and did not want to travel out of town for cycles anymore. Dr Abbasi is the only local RE i have found who is willing to test for and treat immune issues. DH and I have multiple HLA matches; I have Hashimoto's, elevated cytokines and NK cells. All these in addition to severe endometriosis that has caused a severe DOR. Never been pregnant, not even a chemical. If you do get pregnant but miscarry, there is a chance that it is due to immune issues. There is a ton of info on Dr Breverman's website. Intrallipids are done before retrieval, after positive beta and then every 6 weeks, I believe. So not too much trouble in terms of frequency. It’s an IV that the nurse will set up in your home or work place. it takes 3-4 hours to transfuse (sp?). Happy to answer any other questions. |
NP here. PP, have you had the beta-3 integrin test? I have most of the same issues you do--severe endo, DOR, NK cells, thyroid & clotting issues. I too had never had a positive pregnancy test until I combined immune treatment AND a 3 month course of Lupron Depot prior to transfer. Beta-3 integrin problems are highly correlated with endometriosis. |
| OP, I did immune treatment and was successful twice with IVIG. Keep in mind that if your insurance covers it, IVIG may be cheaper than Intralipids. Given your history, I would definitely pursue further testing with Dr. Abbasi. Be forewarned though--she may not be willing to work with you if you don't cycle there. Coram did my IVIG infusions (at home) which I had monthly through the beginning of my third trimester. With insurance, it cost me $50 per treatment. |
One of my cycles was with CCRM and they put me on 2 months of depot lupron. It did nothing for me other than give me horrible side effects. I have pretty much given up that I could get pregnant with OE after 8th failures. Exploring DE now but worried that I still need immune treatment and not every clinic would offer it. |
Yeah, the side effects definitely suck. I decided I wasn't going to put myself through that experiment without being sure we had a viable embryo, so we did CGH first. CGH normal + depot lupron + immune treatment is what worked for us. I did it twice and was successful both times after 4+ years of infertility. Good luck. |

Intralipids are for treating issues with NKs they will not do anything for your TNF if those are also markers you are looking at monitoring. There are other ways that IL works with immune issues though. Check out Dr. Braverman's website on this he recently presented a PPT on all the ways IL is involved in immunommodulation.
I had IL with both of my successful pregnancies. Only had one infusion the first time and many many infusions with my most recent pregnancy. My OB agreed to prescribe them for Dr. Braverman and I had home infusions through Coram home infusion services since they took my insurance. First time was OOP but that was a few years ago and it is much more expensive now. |
