It depends on why you owe.

You also have to pay to set one up: http://www.irs.gov/Individuals/Payment-Plans,-Installment-Agreements

We did this a few years ago and I think we paid a bit of interest, but it was nothing to lose sleep over.

We paid minimal interest. Just call and ask. The people we dealt with were all very helpful.
