No, you don't. I had my 20-week u/s there a few weeks ago, and my bladder was full. The tech teased me a little about how I must have to go to the bathroom. She said once the baby is that old, the image is fine without a full bladder.
Also, the Washington Radiology website says the same thing. |

Nope! They would tell you when you made the appointment if you needed to (I had to have a full bladder for another non pregnancy related ultrasound, and they drove the point home when I made the appointment). |
