What do you call the piece of paper the doctor gives you after your visit that shows the diagnosis?

It's an HCFA form (what you submit to yoir insurance comoany if you see a doctor out of network and they havemt billed your insurance);

Your insurance company then sends you an EOB (Explanation of Benefits) that explains how much they paid toward your visit, what you pay, and why.
