resume update questions

I'm a mid-career professional attempting to clean up and streamline an outdated resume (hasn't been needed since grad school almost nine years ago.
Two questions:
1. Should I still list my college and grad school GPAs? FWIW they were both above a 3.5/4
2. I have been promoted twice in my current position. Should I list only my current title and dates employed by the firm or should I list all three titles held along with corresponding dates?

I would:

1) Not put your GPAs. They're basically irrelevant now almost 10 years later, especially your undergrad GPA which from what I can tell is even more than 10 years ago. Some people say to phase out even putting your school information after a certain amount of years in a career field (10? 15?), in the interest of keeping things concise.

2) List the Company Name as the overall experience, then list each title, the dates, and what unique responsibilities fall under that title. If there are some overlaps, I'd put something like "In addition to the primary responsbilities as a Web Developer, I also execute the following as a Web Manager: blah blah blah". List the most recent title, or what I assume would also be the "highest" title, first.
