are standing desks or moving every 30 minutes "special accommodations"?

This.

I had a doctors note (risk of blood clot...which essentially everyone is at risk of if you are chained to your desk).

fed here: didn't need a doctors note to get an adjustable desktop model. I stand for a couple hours a day, then go back to sitting. It's nice to be able to go back and forth.

Same here. Head of my department thought standing desks were the new hip thing and let us get them after the contracting department approved a few vendors. I only stand a few times a day, but it makes a huuuuge difference.
