Small or medium fits me in the waist and arms, but I need an XL to get around my tatas, which results in batwing sleeves and flapping around the midsection. I gave up on buttondowns, or only wear them unbuttoned over a tank. But I do love the professional look

You have to go custom.

How does Rails sizing work? True to size, or do the shirts run big or small?

I feel like I haven't seen a woman in a white button-down shirt in years.

I was going to say the same thing. Also, does anyone ever find these flattering? Or do people just buy them because they show up on lists of "basics you must have".

I wear a medium normally, but buy a small in Lands End button ups. There’s enough room for alterations to nip in the waist. And with their lower prices, it makes it all work out well.

Lands End seems to work better for short waisted people.
