Power of Attorney Forms with Spouse

Have you and your spouse put in place Power of Attorney documents to designate each other agents? I'm considering doing this for my husband and I but I'm not sure how necessary/prevalent it is.
We've got wills and health care proxies but I've also heard that POAs are important too. We're in Maryland and would be using the new forms the state created in 2010. But I would also like to add a "springing" clause in the special instructions section and would appreciate if anyone has sample wording for that.
