How long it take to get Medicare Card

How long did it take to get your medicare card and notification that social security benefits have been approved after you applied?

Medicare eligibility should begin 12/1/12, applied in mid-September but have not received anything. Applied at the social security office mid-September, they gave me a confirmation number to check status online but the update so far has been that a decision has not yet been paid.
