Has anyoned kid been evaluated for adhd by the school?

My dc was evaluated in grade 3. The result was that they recommend therapy.
Dc did not get therapy but is doing well. (don't ask, too hard to go thruogh insurance red tape)
Can this hinder our chance of getting her into a small private for middle school?

dc is really well. Not the star student you hear other parents brag about, but a good student.

I am asking because the application forms ask this question, and I do not know how to answer.
