I have a doctor's appointment tomorrow for my rising first-grader. Our pediatrician is in DC. Last year, when she entered kindergarden, we showed up with DC forms and had to send them back to the doctor to be transferred to MD forms, which are more detailed, I've learned. I'd like to show up with the Maryland forms, but--can't find them. There is a "health" section at Montgomery County schools' Web site, but the forms all seem to be for kindergardeners.
Is a health form required for first grade? I'm still such a newbie. Thank you for any guidance! |
Go to your child's school and ask for a new form. There should be some 12 month employees there to help you. |
Maybe I don't understand your question.
Why do you need to complete a new set of forms? Your child was in the same school system last year, right? Have immunizations changed? I don't recall doing a second set of forms, but 1st grade was nearly 2 years ago. I definitely didn't do them this past year (2nd grade). |
I called the school and they were not sure that the forms needed to be filled out annually. Since they WOULD know if we needed to update our forms, I assume we don't.
As a previous poster indicated, immunizations haven't changed. Whew. One fewer form to worry about! |
