Bad breath - may be medical condition - would you say something?

I don't want to embarrass my friend by pointing out that lately he's had sulfur-y breath for past couple of days, but I am worried that maybe it's related to a health condition he has. I also don't want to get him overly worried that it means his condition is coming back, but I hate to say nothing if it is related. I know some meds might also cause it, and maybe he's on them.

I have a very sensitive nose and can usually detect very faint changes in scent on myself and others. (diabetic mom= fruity breath sometimes, I can smell a sip of Jack Daniels or beer on someone's breath, cilantro, metallic smells when I'm stressed, etc). If something is changing for the worse, I think he'd want to know but I still hate to alarm him for what might be nothing.
