My daughter's dentist will not fill out her dental form because her appointment was in November. Her school needs it now because she will be 3 soon. Is it typical for a dentist to refuse to do that? |

| Yes- but they cancelled it and now we have to reschedule which may cause a problem with her daycare's compliance. |

Fill the form out yourself and if the record from her appointment is online, print it out and staple it to the form.
I haven't done that for the dental form, but I have done it more than once for the health form. |
Did not know that Daycares take the dentist form. Just reschedule. I am sure the daycare will work around it. It is just teeth, not shots or anything. They can wait. |
| As the previous poster has said, the dentist should give you a form that is dated Nov. If not, make a 6m appointment and show the confirmation to the daycare, they should give you an extention till that date |
