Help! CFA Weekend Nurse and Andrology Lab

I'm surging and we need to schedule IUI for Sunday. I can't seem to find where I wrote down the DC Andrology Lab or Nurse's line numbers. Anyone have those handy? The answering service apparently does not have this information.
