I think it’s probably pain meds or anti-anxiety like Xanax that he got after the assassination attempt. There have been rumors for years that he takes Adderal and that Jr does a lot of coke. So while he doesn’t drink, he could be abusing meds. Or they’ve messed with his brain and this is the result.

He's definitely has the senior mental decline and his latest rants about Biden's decline and that there is "something wrong with Kamala" is the typical projection. Trump frequently anticipates attacks against him and will go on the offensive and accuse his opponent of what he has and then when they try to accurately attack him on the topic, he will point back saying that it is just redirecting what he said about his opponent back at him. He seems to think this is an effective tool as he has used this for three campaign cycles now. Most people realize that he does this and that his attacks are frequently projection of his problems on others as a defensive mechanism.

But he's definitely showing many signs of advanced aging and mental and physical decline. I mean, he has been told so many times how to pronounce Kamala's name and yet he can't remember even for 5-10 minutes. He'll be corrected and then the next time he speaks, he will mispronounce it again. He will go on weird stream of consciousness rants like the old many in a senior home who can't seem to focus. He is easily distracted. Look at how Harris managed to distract him every time in the debates. She's no spring chicken, but next to him, she looks like a recent college grad. It would be a disaster putting him back in the white house. Can you imagine him talking with a foreign leader or diploamt, especially a hostile one? He would never get anywhere and the foreign leader or diplomat would easily divert him on various topics and he would never get to any of the points he was coached to discuss. The foreign diplomats would get him to discuss whatever they wanted and keep him from discussing anything in the US interest. He has the mental acuity of a demented senior and it shows. We cannot afford to have a person like this, who really needs to be provided elder care, making decisions and trying to conduct diplomacy for the US.

He has speech impediments like slurring, mispronunciations, and odd lapses and pauses, that he did not have 8, or even 4, years ago. Take a look at his campaign rallies and speeches from the 2016 election and now. It's impossible to miss that he's mentally a step slower. His diction was always the lowest amongst all presidents (he has the vocabulary and speech patterns of about a 4th grader compared to 9th+ for most presidents), but it's declined even since he was last president.
