I heard a store clerk say it to another clerk when he was serving a lady at the front of the line.

Well, it might have been about her or it could be that they were having an ongoing conversation about something else.

No but I couldn't hear but maybe she said something to the clerk. She looked normal.

Not the OP but why is everyone assuming the entire overheard conversation--or any other part except "loca"--was in Spanish. They could have been speaking in all English except for "loca".
