Reference check for in-home daycare

I am doing a reference check on an in-home daycare where I will be sending my 3 month old. Any suggestions on questions to ask the references?

JH Child Development Center is a home day care located just a few corners of the metro station on the Georgia Avenue (Green Line) station and Columbia Heights Metro Sation (Green Line and Yellow ). JH serves from 8 am-5: 00 pm. Children will be taught and instructed in English and Spanish, creating a bilingual environment where they can thrive, learn and have fun. Learning does not only take place at the gates to provide children with another dimension of the retention of learned skills.

There are some websites with good reference questions for parents who have sent their kids to an in-home daycare provider. When I interviewed references I asked things like:
-How long has your child been with this provider?
-Has your child thrived under their care? How or why not?
-What are some pluses with this provider? What are some negatives?
-Have you had any issues with this provider? Has any other parent had issues that you know of?
-Would you recommend this provider? Why or why not?
