We use Saks -- They run about $150/shirt (get in during the promotional events) and they let you choose from all of their different style shirts (collars, buttons, cuffs, plackets, monograms, etc.). Hundreds of fabrics to choose from and they keep measurements on file (so usually I just go in and pick a few fabrics and have them made for him). The lower priced fabrics (again, the ones in the $130-150 range) seem to dry clean better than the ones in the more expensive ranges (more delicate fabric?). We've ordered about 1/2 dozen shirts every year and that seems to work well for lawyer DH (keeps about 12 in rotation which works for us since his firm does casual Friday that gives us 3 weeks worth of shirts). When the shirts come in, keep them boxed and take them to his favorite tie place to match. Nordstrom also has this made-to-measure service but I don't like their selections as well.
