OP here and thank you for the responses and advice. DD's birthday is next week and CVS does have her birthday on file as part of insurance so we'll try that first.

A few of DD's friends had issues with this because they were not born in the U.S. The birth certificate is in a different language for some of them. |

Ask your pediatrician. Don’t “hope for the best” at a walk up clinic. Be proactive with the medics professionals you know. |

No, they don’t. I’ve never had one that has that info on it-and I now only ever carry the one insurance card, in my name, and it’s good enough for every dr we’ve ever seen for my two kids. My 14 year old was vaccaros a county run clinic held in conjunction with the school district. No one asked for any proof of age. |
