How would you handle this with your nanny?

I think she has the reasonable expectation that her nanny would tell her before, or at the beginning of returning to work in her home. Just like her nanny could expect her to let her know. Nobody is suggesting that she ask for permission.

you should ask the nanny to wear a mask in the house until the results are back (your family may want to also)

As a nanny, I also think it’s odd she didn’t test a couple days before coming back but it is what it is. I wouldn’t expect to be paid every time I myself felt the need to get tested because of my social gathering (from employers, absolutely.)
I wear a mask at work so I’m hoping this helps either way.

My nanny used to do this Crap after years of her working for us, she got too comfortable. In my case we had been gone for 10 days and gave her all that time off work fully paid. The day she comes back to work she suddenly remembers she needs to get her car registered, necessitating another half day off work. Like WTF!? You just had 10 days off to handle that stuff!

She’s trying to milk a few more days off work from you I think.
