IVF authorization denied - doesn't meet "medical necessity"

I had 2 fresh IVFs resulting in 2 sons, and one FET in between the fresh cycles, that did not take. I am overweight, do not exercise, and my diet is crap. I have gone through periods of id al body weight, under weight, overweight, etc, but was never able to get pregnant on my own, despite the fact that all labs are normal. I had seven IUIs before the first IVF , that never resulted in pregnancies.
