I can't find any such policy on their website. Knowing United's crappy customer, we simply called back and booked with someone else. Should I be concerned?

Why didn't he book online? I tested what you posted using flight from dulles to orlando [disney] airport and it came back with a list and you could see available seats before booking. You pay more when you call. You should make sure the route on any airline is normally handled by a wide body jet.

People still call to book flights?

OP here. We called because we had to reschedule my original reservation and add the additional tickets. All were purchased using miles (hence a phone call was required).

The issue with lap children is there are not enough oxygen masks. So in a row with two seats, there are usually 3 oxygen masks.

I travel a lot, including on United, and have never heard of this rule or seen it enforced. My parents traveled with us 5 kids (so a 2:3 adult:child ratio) and never had problems. I am in the Foreign Service, and pretty frequently the spouse of an officer needs to fly alone with the kids while the officer attends to work. I have known parents who have flown solo with 3-5 kids at a time with no problem (other than exhaustion!).
