Can't go wrong with a gap or destination maternity gift card. Does she like massages? I wouldn't do the photo shoot. I'm 24 weeks pregnant and need clothes for me right now than anything!!!!

Good call--if she is a massage person, there's nothing better than a gift certificate at a place she likes.

Otherwise, I'd say a gift card for maternity clothes or a gift certificate to a nice restaurant for a date night. If you know she's really into a certain kind of sweet, say a box of chocolates or a gift card to her favorite ice cream place would also be a hit.
