Have you read the questions on the form?

The humor one is where I completely disagree with you, more than any of the other questions. These kids are 8 year old. They like fart jokes and body function humor, silly, wild stuff, Phineas and Ferb.

If you put a 4 on that section, or even a 3, without concrete, realistic examples that make sense and seem to be true, then you just look like a parent who is looking at their child with a mommy's unobjective lens.

Go with your heart.
Some parents can find shining points from their kids, even looks small to others.
Or you can just say"my kid is average. I don't even know why I waste my time on this"
