We had a similar issue but with Virginia. We just kept calling and finally it got fixed. In our case, they issued it without a mother and only a father was listed. I gave birth in a hospital! Lol I remember being there. I also reviewed it before it was sent off and I was on there. My insurance refused to cover the baby since I wasn’t on the birth certificate. Finally got fixed at month 10.

Miltary doesn't care. My kid's name is in multiple systems in all different ways and no one seems to fix it.

If there is a family registry in the country he is trying to get citizenship, how is he listed in that documentation?

Family registry does not include middle names as they are not commonly used. The issue is the Embassy or Consulate here uses the info on the passport, background check, and birth certificate, and they are looking for them to match. It seems like the best bet is to try one more call to Maryland then send in the form to start the process to reinstate it via the courts. Next up is getting a copy of our DC marriage license and get an Apostille on that. Wish me luck.
