How to avoid confusion with paperwork when your child starts going by a nickname

Hmm. Maybe you don't realize that when you're talking to them, they're typing things up in their computer system. So it's not only when you're filling a paper form that you need to use the formal name, it's any time you're addressing them on the phone or in person.

I am having a hard time understanding the issue with two gym records. Is someone checking gym records when you get a clearance?

I just always use my nickname (Jenny), health insurance and records included. But maybe that goes back long enough it's not an issue. I have never used my legal name. The only places I have my legal name are the bank and government IDs.

NP whose DD has always gone by a common nickname and who also has never had this issue. I agree that there's got to be a user error happening.

My DD official name is Kate and she is often asked to use her legal name on forms. People assume she is Katherine.
