if you're new and embarrassingly late to the face game ....

Late 50s, currently most glaring issue is deep marionette lines, more pronounced with recent weight loss. Next is probably just lots of spots (age and others I think), light freckles, etc, on my face. Preference for Tysons/McLean/Reston area. Thanks for any guidance.

Why did you wait so long? Like everything was fine up until today? Usually women are at least doing laser treatments and botox in their 30s/40s? Have you always felt this way and suddenly came into money? Recently divorced?

I had fillers for my marionette lines and my face still looks better years later. I didn’t have them redone. They can be done in a very subtle way. Just don’t do lip or cheek fillers. Those look bad (in my opinion!).

You might need a mini face life. They look very natural now. Don't do any fillers even if it might be the only thing to fix your marionettes. I would wait to see if you like the laser/botox/tret first. It might be all you need. Exercise will tighten loose skin too.

why all the "no fillers!" comments, when my google research seems to suggest fillers are the answer to marionette lines?

Because fillers always look bad.

Bad fillers look bad, good fillers aren't noticeable except to the person who received them. Go to a true cosmetic derm (not the person who does your skin exams).

Most med spas have “events” where the treatments will be on sale. Try all this and then move up to neurotoxins and fillers if necessary. Find one person whose work you trust and tip her well and always go to her.

I would go to a dermatologist office for microneedling or IPL. Over a medspa, unless it is higher end one.

I go to Dr Tamjedi. While I think she has gone overboard on her own face, she is super restrained on the fillers she has done for me. It is very hard to get an appointment with her though.

Moksha at least is a medspa.

I like SkinDC in Arlington.

Once you figure out what services you want (maybe an ablative laser followed by a series of microneedling + botox + tret) you can shop around a bit on price/location/etc. Anything more serious than microneedling you should definitely see a proper derm/plastic surgeon.

Wear GOOD sunscreen every day. Reapply. Wear sun hats.

Do a CO2 laser
Get Botox in all the obvious places
A little bit of filler for the marionette lines only
Makeup consultation
Color and highlights

I would go to an excellent dermatologist before a medspa but that's my opinion. Ask their opinion. Overall it is typical to begin a skincare routine using a retinol, ask the dermatologist for a script like tretinoin. I would start with botox around eyes, head, brow line. You can consider microneedling which increases collagen and improves skin quality. I've been pleased enough so far with these things but I lean toward liking a more natural look so I avoid anything with my lips or filler. Though these can be done well. I like Dr. Ann Rothman.
