Teen feeling light headed last night

If the condition persists I would bring up syncope to your doc, one type is POTS. Another type is when you feels dizzy or faint after moving from a sitting or lying to standing position

The change in appetite is new. The ADHD meds are not. He has near syncope. His BMI is in the 17s. He should go to the pediatrician.

He has POTS. Increase fluids, electrolytes, salt, compression, and have him change positions slowly.

What you’re describing is POTS.

Get him SALTY snacks. It will help him hold onto water that he drinks.

At that height, and such an emaciated weight, plus taking what is basically speed, what do you seriously expect to happen?
He needs to eat more and get his BP in line. It's probably low due to being so skinny. At his height he should be around 160 at least.

Yes, but he’s growing now, so the meds/appetite combo has changed. Talk to the ADHD prescriber about the change in appetite.
