DC has a website where you can look up info on each licensed facility and see PDFs of annual & monitoring reports for the facilities, including deficiencies identified by inspectors.

Our daycare center replaced the director because of record keeping deficiencies identified by DC. I'm assuming there were other issues too, but the record keeping stuff is public domain and explains why they were removed.
