We didn't have dd wear a helmet on her trike -- it never occurred to us and since she didn't really like it, she never got going very fast -- but she has no problem at all with knowing that she must wear a helmet on her two-wheeler. In fact, she likes the helmet more than the bike.
It doesn't really matter how fast they're going. If they fall over or backwards even from that low height, they can sustain a serious head trauma. Since the downsides are the cost of a helmet and the potential dork factor and the positives are preventingpossible brain injury and creating good habits, I think the helmet wins.
We got one for DS because he rides on city sidewalks and occasionally we have to cross the street. I would also require a helmet to make it a habit. DS now automatically grabs his helmet whenever he rides his trike or scooter. I'm glad we instilled this habit in him when he was young.
