I’ve really struggled with the same decision but have ultimately decided to self-insure, at least for now. I just don’t feel like the value of the potential payout is worth the cost.

Uhhh…I don’t think so. $500/day sounds about right. $250 for the long term care facility alone seems like a steal but all the labor with cleaning, etc…absolutely would run fairly high. If you think cleaning people should be paid commensurate to care for old people….be prepared for the absolute bare minimum job (ie: I took your trash out what more do you want).
