where did the doctor ratings go?

Some time ago, perhaps as long as a year ago in fact, I upgraded the website software and the doctor ratings stopped working. I removed them with the intention of debugging and fixing things when I had time. Then, I noticed that nobody seemed to have missed them. In all that time, you are only the second person to ask about them. I really wasn't a very frequently used resource here and I probably won't bother to restore the feature at this point.
