He’s the most powerful person in the world, has loyal followers, has been President twice, has dedicated family members, has an attractive wife, and has billions of dollars. Why does he not have the capacity to be content and happy for one minute?

Because all he ever wanted was the love of his father, and he never got it. He's suffering from malignant narcissism. There is no cure for that. He can never be happy, but he has spent his entire life making others unhappy in his drive to acquire wealth and fame to fill the unfillable void.

He has all that and still no one loves him. If he were just some old dude ranting in a nursing home he'd be the one with no visitors. The people around him are flies attracted to poop.

I want to say if you went back in time and gave him love as a child. Something definitely went horribly wrong there. But also, plenty of people have crap parents and don't end up like him.

Maybe if there had ever been consequences for his bad behavior. We've learned that a rich white guy can grift through life and have no consequences.
