| So my DH and I have been cleared with fertility tests and given the diagnosis of unexplained infertility. RE told us we could go straight to IUI (unmedicated) or keep trying on our own for a few months and then IUI. Our original plan was to keep trying on our own for a few months because we thought this month was out due to DH's business trip and next month is out because we planned a vacation. Turns out I ovulated WAY later than I expected -- last night, cycle day 24. My DH is on a plane on his way home. So we could just try on our own according to plan or I could go in tomorrow morning for an IUI. I don't know why but this whole ovulating late thing and thinking I wasn't going to ovulate has freaked me out. And now I think I should go in for the IUI. Can't discuss with DH b/c he's in the air at the moment. I am sure he'll do whatever I want. But I need help making a decision. I know it's stupid to ask complete strangers but maybe your thoughts will help. I fear regretting either decision. |
| So you already ovulated right? Or did I read that wrong. If you have already ovulated, tomorrow morning will be too late for an IUI. |
| OP here, I got a positive on an opk at 11:30 last night. So I would ovulate within 48 hours. I was surprised they said to go ahead with it and they intend to do an u/s prior to insemination if we go that route. So maybe hold off b/c timing is not exactly on point? If I hold off though it will be December before we can do IUI. |
| If you got a positive OPK last night, you can ovulate in 24-48 hours. So there's a good chance that IUI tomorrow morning will be too late. If you are paying out of pocket or you have a limited number of |

A limited number of tries under your insurance, I'd wait for a better timed cycle. If cost is not an issue, there's no harm in going ahead with the IUI tomorrow. |
| I think it's too late for an IUI this cycle. Sperm needs time to travel to the fallopian tubes from the uterus and go through some special process to be able to penetrate the egg (apparently takes quite a few hours according to what I read). Today would have been a better day. So I would try naturally this cycle if I were you. |
| Sorry but if it's too late for an IUI isn't it too late to try naturally as well? I mean, I get the reasons not to shell out the $$ for IUI. But just curious. Thanks. |
| If you're unexplained, and presumable your DH's counts are good, I can't see how an unmedicated IUI is going to increase your chances of success. Maybe by 5% at the most? Not sure you want to spend your money that way. |

| If your OPK was positive last night, it's not too late to try tonight because if your egg is released say this evening, it will survive for at least 12-24 hours, enough time for the sperm to reach it. If you do an IUI tomorrow, an egg might die by then. |
| I'd really investigate the issue of why your RE won't let you do a medicated cycle yet. Perhaps s/he thinks more time naturally would also potentially work? If it's raelly true that unmedicated IUIS only boost your chances by 5%, it seems strange to me... and, given my own suspicious personality, I'd wonder why s/he was dragging things out. |
| My RE also thinks that an unmedicated IUI is the way to go for me (we have a male factor) if I wasn't willing to jump into an IVF. Since I am 38, I decided not to waste my time and go into an IVF. |
Yep. Even 24 hrs is a stretch for egg life. Most eggs begin to degrade around the 12-hr mark and are not viable by the 24-hr mark. The reason the window for conception is a couple of days is because the sperm can survive that long, waiting around for the egg. So you need sperm waiting in the tubes when the egg is released if you're going to have a good chance. IUI shortens the journey considerably as you bypass the cervix. Normally, medicated IUIs are timed to coincide exactly with ovulation - you trigger and the insemination takes place ~36hrs later, which is when the vast majority of women will ovulate after a trigger. There is a little more variability in the timing of ovulation from a natal LH surge as opposed to trigger shot, so it could be 24-48 hrs after the surge. |
| Thanks everyone. We opted not to do the IUI this cycle. I am hoping we are covered as we bd'd monday morning, last night, this morning and we'll try again for tonight. My LH surge was Wednesday at 11:30 pm. So fingers crossed. I also am taking prometrium for the first time. I had a total melt down last night. Wailed for 2 full hours. This is just so draining. |

So undestand you, OP, regarding the draining emotional aspect of TTC business. I am in the same situation.
Re: prometrium. Since you have unexplained infertility, why did the RE put you on it? Also, what is the dose are you taking and how many times a day? |
