2 28mm follicles?

Anonymous
So i had ultrasound today that showed one 28mm follicle on each ovary and nurse said will probably have me trigger tonight for timed BD. Took clomid days 3-7. I quickly looked online and there was some talk amongst other TTC people that 28mm is too big and not good. Any feedback on this issue or experience?

28mm is big (maybe "too big") for IVF, not sure about timed BD.

OP here again. What is "too big"? I mean what does that mean? Too big for what? Why is it so big-the clomid? Should I have triggered earlier? I am confused. I will say my ovaries are throbbing

Too big may mean that your eggs are post-mature or overly ripe. I don't know if this is the case for you, but I know that for IVF if you have follicles that big, if they retrieve eggs at all from those follicles, they won't be good anymore.

I agree those may be too big & overly mature. Was this the first time you went for monitoring and they were already that big, or did the RE let them keep growing? I always triggered with smaller sizes than that - depending on what cycle day you are on, you probably should have triggered earlier.

I know that for Clomid, the recommended sizes are bigger, so you may still be ok. Next cycle (if you need one) you should make sure to come in for an earlier ultrasound though.

OP here-Yes I am on clomid and day 12 of my cycle. This is the first time I went in for monitoring. I tend to ovulate on the earlier side, but figured they knew what they were doing when they told me to come in today. Guess I was wrong. The nurse said it was that big because of the clomid though and did not act like it was a big deal. I am waiting for them to call me to tell me what time to trigger tonight and will ask her these questions.

I was on injectables and had successful pregnancies when triggered at 18mm and one time at 26mm. I also heard that with clomid, they trigger when the follies are bigger. I would imagine that you spontaneously ovulate if they got too big. I also think that egg maturity applies to a time period as opposed to follie size. For instance, if OP was at day 19 or 20, then egg maturity could be an issue.

OP again. Just talked to nurse and they want me to trigger tonight. She said the size was great and what they expected/wanted on clomid at this stage. Hope she is right!

I did 4clomid cycles and triggered with smaller sizes.

From what I've read, on clomid cycles, you trigger at 17-26mm and on injectibles you trigger at 16-20 mm. Yours are on the high end even for a clomid cycle but if the nurse said its ok, hopefully it is!

Good luck!

I would like to know how you made out with this? I had a 28mm follicle yesterday when I was surging.

OP here-I don't know yet PP. I went in for bloodwork today, a week after the trigger shot. I assume they are just checking for progesterone levels, but I am not really sure since I am only 6 DPO. I am sure I ovulated because I am charting. I think I ovulated on Sat, even though I took the trigger Fri night and I have been wondering if it is because the follicles were so big already and I was basically going to ovulate on my own at that point anyways. We shall see. I feel nothing as of yet, but not sure I would anyways.

OP-Well good luck to you!! I went for my 2nd iui today. I took femara this cycle 10 pills on day 3! I did surge on my own so I bet you were right about to as well. The last cycle I responded to femara was a 28mm follicle as well and the doctor thought I was going to surge on my own but I had insisted on the trigger(bfn). Did they tell you what your E2 level was? Oh and I concieved my daughter(3 years old yesterday) on a clomid cycle with timed bd. Good luck and keep me posted!

OP here again-PP-I too have a 3 yr old We have been TTC #2 for well over a year now. I got pregnant with DC without any assistance the first time and in the first month we really tried. So, this has been an eye opening experience for me. They did not tell me what my E2 levels were on Friday. They did call me and tell me my progesterone level was 32 and they were happy with that number and to come in the following Friday for a pregnancy test. That was about it. Good luck to you with your IUI. That is the next step for us and we are trying to decide if this cycle does not work if we want to do one more with timed BD or move straight to IUI. We shall see. Good luck to you and I hope for the best for all of us here.
