Federal Carefirst BCBS denied my IVF coverage

Has anyone else dealt with this? I had a frozen embryo transfer and coverage was denied under “710 - This is not a covered service under member’s plan.” I specifically chose Carefirst because it includes IVF coverage. Has anyone had this issue and fought back successfully?

What! Maybe your provider was out of network?? Curious what you find out. I too chose this plan for the IVF coverage and was thinking of a transfer this fall. But my in network RE did a preliminary confirmation of my benefits and showed it covered after deductible…

Do you have a prior approval in place? Did you get BCBS Standard?

hello. i was denied on the first try and only given permission to try iui three times. I have already had one live birth from IVF with Aetna. But carefirst did not care. Somehow we tried to get auth again after it expired and they approved three round of ivf. I also wrote them a letter about my age ( 43) and natural attempts at IUI. Or have your doctor write medical necessity.
