It means consistently.

Is Participation (P*) and Assignment Completion (AC*) considered a learning skill?

It was just a printout that my kid gave me and there was nothing at the back. I just checked and found the link above.

The official report card absolutely has the codex printed on the back. Even in HS where they only upload the doc there is still a page for the codes.
