Chronic Illness Care/Oversight at Home--Visiting Doc/Nurse Recs in North Bethesda/Rockville?

Hi everyone, I'm trying to find someone who can visit my mother once a week or so to check up on her medically. She has a team of doctors, but they treat her for acute issues and there's not a huge amount of coordination and no check-ins unless things get urgent. She has several serious health issues, including congestive heart failure, and needs more ongoing attention. Ideally, I think this would probably be a nurse who comes to the house on a regular basis. We have used nursing services after hospital stays, but those services run out at a certain point and we need something that will last as long as necessary. I am happy to consider private nurses (instead of agencies) if there are any who do this kind of work. Many thanks for any suggestions!

What kind of care? Checking vital signs, administering meds ?

Thanks for responding. For the most part, she does not need help with vitals or meds. She has a companion living with her who can do that, but she can't assess her medically. She needs someone who can address issues/symptoms as they arise, coordinate with doctors when necessary, and also give her some peace of mind. Her doctors/specialists are good, but they don't check up on her. I have been looking into at-home nursing programs but so far none serve her area. I've been calling geriatricians and even concierge medical practices. So far, all answers are negative: we don't serve that area, the practice is full, etc. I'm beginning to think that an independent medical professional--ideally a nurse practitioner?--who works on his/her own may be the only option as we have not had luck with agencies.

I'm just struggling with what "assessing her medically" means on a weekly basis. You could probably hire a CNA to take vitals but otherwise it's tough to understand why she nor her companion can't call a doctor or go to the ER if there is a concern. An NP that essentially gives her a weekly check up seems like overkill. What are her chronic illnesses?

Gilchrist, as in the Baltimore area hospice company, offers a non-hospice level of care called "Elder Medical Program," for house-bound individuals with at least 3 serious chronic illnesses. This is covered by Medicare, and includes a nurse visit weekly, 24/7 nurse line, and coordination among the Gilchrist doctors, social workers, and nurses.

Do a search for Elder Medical Program, in your area. Maybe that could help.
