| It depends on the situation. Sometimes the person in charge of the project should be put first, not the leader nominally overseeing the group. |

No, I don't let people stand on ceremony that way. Sometimes if there are four men and one woman, I'll put the woman's name first, because #girlpower though.

In the "To" line yes I go by rank if I know it, because some people care. |
